Cerebral Palsy – a talk and discussion with parents in Block Resource Center, Palakkad Kerala

It was so nice to meet and talk to the parents about #cerebralpalsy and #developmentaldelay in #children at the #puthur #brc in #palakkad yesterday

Stress was on the need for #earlyintervention and adherence to #treatment especially #physiotherapy. The challenges parents face while continuing care is an eyeopener everytime I hear them out.

As in most diseases, treatment challenges in #cerebralpalsy is more to do with #social, #familial and #accessibility issues than actual lack of desire.

Most parents are willing to face the #financial challenges if it means the child will improve. Many are disillusioned with existing treatment methods and fall out because of slow progress, lack of motivation from professionals around and also pure financial pressure. Many have siblings who are healthy and would like to divert their limited resources to that child. This is such an unfortunate choice , but many parents make it.

Such talks and interaction are a wonderful opportunity to break the ice, motivate and also clear their apprehensions on the treatment of cerebral palsy.

A big thanks to Puthur BRC for organising this meeting. More to follow 🙂

Attaching below a few pictures and also a short video which the BRC officials had shared.

Cerebral Palsy : Treatment Possibilities , a talk

Excited to address the #Calicut #Orthopaedic Club this Friday and speak to stalwarts, teachers and trainees on #CerebralPalsy and the #orthopaedic treatment possibilities.

I will speak on the need for early detection, treatment and also about the newly released hip surveillance guidelines.

The trainees and general orthopaedic surgeons need to understand the importance of early referral and the benefits of early treatment.

In India with its exploding healthcare facilities and young population, we need to be aware, equipped and future ready when it comes to cerebral Palsy care.

Today I will be speaking at the inaugural function of this new effort by Karunya MVRC at Palakkad Kerala.

Last year the team at Karunya broke ground and made a sensor driven, no touch hand sanitizer dispenser. This was made by differently abled students.

Unfortunately COVID pandemic raging again meant that all events and celebrations had to be toned down and postponed.

Will post again on updates in a while …

Last year these specially abled children designed and built a sensor driven sanitiser dispenser.

Will post more pictures later today after the event.

#Onam, a festival in #Kerala to honour erstwhile King Bali and Vamana, the Avatar of #Lord #Vishnu ~ A Poetic Symbolism

Childhood in Kerala growing up was a pleasant time. There were no mad rushes, lot of rain and then, Onam. Onam celebrated on the Thiruvonam (Shravana Nakshatram) star day is a festival that is steeped in Hindu Puranic lore. The story involves the great King Bali (sometimes referred to as Maha-Bali ) and Vamana ( an Avatar or Incarnation of Lord Vishnu ).

The Onam / Vamana Jayanti Story, What it was and what it has become …

The story as in many Puranic lores is of the classic clash of a devout, efficient, benevolent King who is an Asura ( literally, one is not-a-Deva ). The benevolent Asura King usually causes flutter in the Deva Kingdom ( It’s king being Indra ) forcing the Gods to incarnate, kill the Asura and rescue the Devas. The story of Onam here follows this same storyline; only as in the other stories the symbolism is hidden, deep and often missed when one keeps celebrating festivals superficially. In the case of Onam there has been a systematic over-simplification of festival. I remember in my childhood we use to keep Trikkaakarayappan (Vamana) and Mahabali (sometimes called Maveli) as little clay pyramids in midst of the flower arrangements (Pookkalam ) we used to do in our home portico. Over the decades, this has all but disappeared from the celebrations. There is hardly any mention of Vamana anymore, no clay pyramids, many do not know Thrikkakarayappan. Some years back there was even a needless political controversy when Vamana Jayanti (Birthday celebration of Lord Vamana) wishes were given on even of Onam. The simple fact is, Vamana Jayanti is celebrated in our households and many Vaishnava homes just as it was earlier. It is just that with increasing commercialisation, securalisation of Onam and making it a harvest festival Vamana Jayanti seems to have been forgotten. This is unfortunate as Vamana Jayanti is Onam itself. There is no Onam without Vamana as much as there is no Onam without Mahabali.

In all this pell-mell of making it more secular, more harvest and less Hinduism, more commercial and less spiritual, more literal (Rich Noble Asuric King subdued by Brahmin Vamana and rudely pushed underground ) a narrative that suits the more left-leaning, liberal-leaning of the political spectrum has been woven into this mixture.

It is important that ancient India, it’s traditions cannot be painted with same brush. It is also important that one goes in to the original Sanskrit texts to reference stories of Mahabali and Vamana and read for oneself the narrative.

The Onam / Vamana Jayanti Spiritual Symbolism

Now is the time to resurrect the Vamana Jayanti Tradition, It’s symbolism, the true stature of MahaBali the Noble King. My poetic attempt at is below…

What is Onam without Thrikkakkarayappan , the Vamana ?

Symbolism Note : The reference to “Wild Sugarcane tasting ripe and sweet” is a reference to a Saivite Saint from Tamil Nadu, Pattinathaar or Pattinathu Swamigal. He was told by the Lord that he will attain Samadhi when the wild sugarcane tastes sweet. He used to go to the grove and taste sugarcane everyday to see if the day of his Moksha had come.

Free Medical Camp for Paediatric Orthopaedic Deformities, Cerebral Palsy and Neurological disorders at Palakkad, Kerala

So happy that we’ve had a very successful #PediatricOrthopedic and #CerebralPalsy #Medical camp today at #Palakkad, #Kerala. This is one of a kind medical camp, parent education drive and cross speciality clinic that we’ve been hosting at Palakkad since 2014.

This is the 6th year of my setting up Paediatric Orthopaedic clinic here at Palakkad Cooperative Hospital, Kerala and patient awareness of the problem of Cerebral Palsy, ClubFoot has increased tremendously over these years.

Sevabharathi volunteers were as usual wonderful and committed. None of this would have been possible if not for the commitment of Biju Bhasker and his team of LifeCare Physiotherapy Centre at Palakkad, Kerala. He has been the main organising force behind this over the years.

Today we had a Pediatric Neurologist Dr. Velmurugan and Pediatric Gastroenterologist Dr. Jaganandan join us for the camp. Both specialists are from Coimbatore. It was so nice of them to take time out and join our efforts.

With God’s grace we’ve been able to consistently raise the standard and tune the camp to make it more cross specialist so that patients can consult multiple specialists in the same visit. We try with every camp to make it better. I was discussing with Udayasankar CN of Sevabharathi, Palakkad that next camp should be much better in many ways.

I could identify some children who will require surgical procedure, plaster casts, Botox injections which could be funded, if approved, via the planned free surgery program at Palakkad for the #Bone and #Joint Day of #IndianOrthopaedicAssociation in the first week of August.

Attaching below some pictures of the event.

Pediatric Sports Injuries in #India – the #Palakkad ,#Kerala Story

The small town of #Palakkad #Kerala state may be one of the biggest contributors to #Indian #Athletics. It’s a town predominantly know for its large expanse of rice fields and farming community. In the past athletes like Valsamma, PT Usha have trained here and succeeded. Recently PU Chitra bought laurels to this small town. Many children here look upon these heroes as they train daily.

Recently Palakkad got it’s own first synthetic athletic track and provided a field where young athletes could be groomed better. Several promising athletes are in training in this otherwise rustic, tucked away place. Athletes who’ve successfully represented India at the Asian and Olympic level belong here and have trained here.

Most of these athletes use what is locally available in terms of facilities, gears, coaches to train themselves. The fact that until recently they didn’t even have a change room where they could change themselves into or out of the sports gear after coming from their schools or colleges speaks volumes of the lack of a system in place. Many are from very humble backgrounds. Driven by the need to succeed, as it’s very often the only glimmer of hope from poverty.

Things are changing though. Changing rapidly. As I train and interact with these children while setting up my Sports Injury Clinic here, I realise a renewed vigour in children, coaches and parents where they are unafraid to take up their needs with local authorities and get it done.

With more training facilities promised and the track and field itself being in the process of a long overdue upkeep and upgrade as I type this, it is certain that with children in #India taking to #Sports early and training intensively , #Pediatric #SportsInjuries will become more common. Though there is difference in the pattern of these injuries from those in the west, nonetheless, these required specialised care.

Just in last week I have examined and treated children with old hamstring tears who’ve not rehabilitated well, old ankle injuries with pain, back pain due to stress and renal calculi as a result of training in summer heat with very poor hydration. None of these children received adequate on field or off field medical attention. All of them were not aware of the type of injury they are having and its potential for causing premature career collapse.

I have realised that along with adequate sports injury prevention counselling, these children require counselling for sports nutrition and diet tailored to the sports they are participating. It is sad that while these could be given, not many maybe able to afford such a nutrition on daily basis. Mr. Haridas the coach of the Olympic Athletic Club here at Palakkad is so kind. Everyday he brings these kids mangoes from his farm. It’s little things like these that bind the coaches, parents and kids together and goes beyond simple trainer-trainee contract. The fact that he does this training free of charge is really something laudable.

This is the story of sports in India….

We need better facilities, coaching , gears but most importantly these children need better nutrition. They also need better awareness of injuries as they push themselves to greater heights.

Aniruddhan with his father. His latest fracture a minor blow in his quest

How I met Aniruddhan and his father He came in to my Outpatient Paediatric Orthopedic Clinic at Palak kad, Kerala, brought by a common friend who’s son is also a roller skater. Aniruddhan suffered a fall while practicing and had fractured his right wrist. With a competitive sports time away from tracks and practice is time somebody else could spend to improve and get ahead of you. I advised his father that since the fracture could be immobilised in a cast with artificial material giving it some strength that a regular plaster of paris would, he could get back to practice with his cast on the same day. We could always do weekly check-ups to see whether the cast is becoming loose or if the fracture is moving and alter treatment plans accordingly. Aniruddhan’s dedication is so much that he jumped at the idea. His father, who has been encouraging him also advised him to do the same.   This simple act of dedication and ability to tolerate an adverse event inspired me to ask more about him. What I heard from his father about him merited a small story. I cannot divulge many of his early struggles because of patient confidentiality, but what ever his father found non-sensitive I am sharing with the world to inspire other kids (and parents) who dream. Hopefully it would make them dream bigger and reach higher.

His story, a simple inspiration

His story of how he overcame hurdles in his early education and learning difficulty by complete dedication to his sports and earned peer recognition is wonderful to hear. Not only did it help him achieve laurels in sports it also helped him overcome his difficulty in learning and progress there too. I suppose sports does that to people. With intense training in balance, coordination, fitness, struggle against odds, facing the exhilaration of success and depths of failure character builds.

Doubtlessly.

Tough Life of practice, dedication to sports and educationAniruddhan is now 11 years, in 6th class has been religiously getting up at 3 am every morning for the past 4 years. His father accompanies him to Coimbatore, a nearby town about one and half hour by road, where there are facilities for roller skating practice. After practicing for 2 hours he returns at 8 am and then does his full days school and then in the evening does his road practice on Roller Skates.  It is no wonder then that Anirrudh is a National Level Skater and deserves every bit of encouragement he can get. He sustained an unfortunate accident and fractured his wrist last week. Aniruddhan has a cast on him now, but his spirit has not been dampened. I have recommended to practice roller skating with his cast on !Aniruddhan’s story has just begun. He is at the Junior National level now with mile of hard training struggle ahead of him to get ahead of the tough competition. However, if what I have seen is the attitude he keeps then his success is sure, in time. He has now moved to inline skates and is entering the world of elder children with some tough competition ahead of him. I am sure that with his dedication to the sport and will to practice in spite of odds he will shine through and bring laurels to his loved ones and country !The story of Aniruddhan is one among the many who have come out of all odds and made it to the top. Watch this space for more inspiring stories of child athletes, their battle against sports injuries towards victory.
